In medicine and epidemiology, the effect of classification uncertainty on apparent test performance is repeatedly referred to as „Information Bias“, „Misclassification Bias“ or „non-differential Bias“ and has other names in other fields [8-10]. These terms refer to the fact that as classification uncertainty increases, there will be a widening gap between the actual performance of the test and empirical measures of test power such as sensitivity, specificity, negative forecast value (NPV), positive prediction value (APP) or area below the receiver operating line (ROC AUC). It has been known for many years that the imperfection of available comparators is a source of difficulties in evaluating new diagnostic tests [11-16]. Recent literature describes a number of examples where the use of imperfect comparators has led to complications in evaluating the performance of new diagnostic tests for diseases as varied as carpal tunnel syndrome [17], kidney damage [1,18] and leptospirosis [19]. Generally speaking, a known uncertainty corresponds to a well-expected erroneous classification rate. .
